What to Bring:


Below are the registration forms needed to enroll your child in Howell Public Schools. The astericks represent forms you need to print off, fill out and bring with you. The rest of the information is parent/guardian responsibility to obtain from the previous school or your files, we will make copies and give the originals back to you.

Everyone:

1 - Registration Form*
2 - Residency Form*
3 - Certified Birth Certificate
4 - Immunizations Record
5 - Proof of Residency
(driver's license + utility bill, rental or purchase agreement + residency form)
6 - Legal Guardianship Papers
(if not parent)
7 - Name/Address/Fax/Phone of previous school attended

 

1st - 12th, in addition to items above:

8 - Records Request Form
(select K-8* or 9-12*)
9 - Discipline Form*

 

High School - in addition to items on left:

10 - Bring transcript from previous school

 

Special Education - in addition to items on left:

11 - most recent IEP

 

Kindergarten / Begindergarten:

1 - Registration Packet 08-09* (checkoff sheet is in packet)

2 - Vision Screening Record

 

08-09 school year kindergarten/begindergarten:

if possible please register on or before February 26, even if you do not have the vision screening or immunizations completed yet
Please visit the Technology Office / District Registration Office at 411 N. Highlander Way, Suite E, to register your students at least two (2) days prior to the date you wish your child(ren) to begin. This allows for the student information to be entered into the school database and class placement / scheduling of your child(ren) to be completed before their start date. We will also have you notify / arrange transportation at that time. If there are no openings at your home school, your child(ren) may be transferred to another school within the district. If that happens, transportation WILL be provided. Your child will be placed in their home school the following school year.
